Initial Comments:
Based on the findings of an off-site follow-up survey completed April 22, 2022, Community Care Inc. was found to have corrected the deficiencies cited under the requirements of 42 CFR, Part 484, Subparts B and C, Conditions of Participation: Home Health Agencies. The deficiencies were cited as a result of unannounced on-site State Re-licensure and Medicare Re-certification survey completed on January 13, 2022.
